Cytolytic Vaginosis: When “good” bacteria go too far

The Australian health information service healthdirect, says self-treating thrush is generally only safe if you have had it before, treated it successfully and are sure of your symptoms. It recommends seeing a doctor after 4 or more yeast infections in a year. Cytolytic vaginosis is one reason that advice exists: it is a little-studied condition that can mimic thrush closely.

You will not find cytolytic vaginosis on the major Australian health sites, so this article leans on published research and says clearly where the evidence runs out.

Quick answer: Cytolytic vaginosis is a debated diagnosis where a vaginal sample shows lactobacilli, the usual "good" bacteria, in very high numbers together with damaged lining cells. It brings thrush-like itching, irritation and white discharge, but antifungals are not expected to fix it. Diagnostic criteria remain unsettled.

What is cytolysis, and how is it linked to cytolytic vaginosis?

Cytolysis simply means cells breaking apart; in this context it refers to fragmented cells from the vaginal lining seen under a microscope. When that finding sits alongside abundant lactobacilli, and yeast, trichomonas and bacterial vaginosis have been ruled out, some clinicians call it cytolytic vaginosis.

Doctors Leonard and Laurence Cibley gave the condition its name in 1991. A 2023 scoping review in PLOS One, covering 64 studies, describes it as controversial and says its validity still needs confirming.

How does it feel compared with thrush?

It can feel much the same as thrush. Studies describe white discharge, irritation and itching; one also recorded pain during sex. Healthdirect lists itching or burning around the vulva and thick white discharge as typical thrush signs, which shows how easily the two can be confused.

The review found microscope findings were better than symptoms at telling them apart. That is the practical point: if repeat thrush treatment is not working, a swab may give a clearer answer than another round of cream.

Is cytolytic vaginosis serious?

Current research does not describe it as serious, although symptoms can be distressing when left untreated. Some studies reported links with fertility or pregnancy outcomes, but the reviewers rated their risk of bias as moderate to high.

If "cytolysis" appears on a cervical screening result, ask your GP to explain it. The review notes that screening-based studies reported the lowest rates of the condition and that two studies found no link with abnormal cervical cells.

Is a period cup OK if you have cytolytic vaginosis?

No research we could find has studied menstrual cups in people with this condition. Within the review, 6 of 35 treatment papers advised giving up tampons, arguing that menstrual blood is alkaline and raises vaginal pH. Because that idea was never tested on cups, the honest answer is to talk it through with your GP or gynaecologist.

On treatment more broadly, bicarbonate of soda sitz baths or rinses are the approach most often suggested, though mostly on expert opinion rather than trials. Healthdirect lists douching among things to avoid, so do not start internal rinses without medical guidance.

When to see a doctor

  • It is your first time with thrush-like symptoms.
  • Over-the-counter thrush treatment has not worked, or you have 4 or more episodes a year.
  • You are pregnant or breastfeeding.
  • You have lower abdominal or vulval pain, abnormal bleeding, painful sex or fever.
  • Discharge looks or smells different from your usual (see healthdirect's vaginal discharge guide).

Frequently asked questions

What pH is normal for vaginal discharge?

Healthdirect says normal discharge in people of childbearing age is slightly acidic, with a pH of about 3.8 to 5.0, which helps protect against infection. A home pH strip cannot confirm cytolytic vaginosis, because researchers rely on looking at a sample under a microscope rather than on pH alone.

Could my probiotic supplement be causing this?

The evidence does not answer that. A 2023 review of all the cytolytic vaginosis research it could locate did not mention probiotics, so there is no basis for saying they cause it. If you take a lactobacillus product and have ongoing symptoms, include it when you describe your history to your doctor.

How common is cytolytic vaginosis in people with symptoms?

Figures are uncertain. Pooled across ten studies, the median was around 5% of people with vaginal symptoms and around 22% of those with recurring symptoms. The authors describe this as limited evidence, with almost no data on people without symptoms, so treat these numbers as rough guides.

Will antifungal cream help?

It is not expected to, because cytolytic vaginosis is not caused by yeast. Some papers in the review even suggest stopping antifungals once the diagnosis is made. That is why repeated thrush treatment without relief is a signal to see a doctor and ask for a swab.
